Filing for bankruptcy under Chapter 13 demands a debtor to file the various documents defined under numerous Schedules to the federal bankruptcy court along with the filing fee and the administrative fee. Debtor’s Certification of Employment Income, Chapter 13 Plan, Declaration concerning debtor’s Schedules, Individual Debtor’s Statement of Compliance with Credit Counseling Requirement under Exhibit D, Voluntary Petition under Exhibit C, are the other documents to be filed under Chapter 13. Over and above these documents, the debtor is to submit Statement of Current Monthly Income and Calculation of Commitment Period and Disposable Income (Form 22 C), Statement of Financial Affairs (Form 7), and Statement of Social Security Number or other Taxpayer Identification Number to aid in the verification of the details on the last tax payments made by the debtor.
